Home Supports (HS)

Provided to consumers over the age of 18 who want to continue to live with their family. The service meets both personal care and habilitative needs. 

Residential Supports (RS)

Provided to consumers over the age of 18 who can no longer reside in their family home and the service meets both personal care and habilitative needs.

Home & Community Supports (HCS)

HCS is a habilitation service that enables the client to acquire and maintain skills that will allow him/her to function with greater independence in the community. This service can be delivered in a participant’s private home or in a variety of community settings to which the participant chooses to attend.

Personal Care Services (PC)

PC services include support, supervision and engaging participation with most activities of daily living such as eating, bathing, dressing, and personal hygiene.

Enhanced Personal Care (EPC)

EPC also includes support, supervision and participation with daily activities but is intended for participants who have intense medical or behavioral needs

Respite

Respite Care provides periodic relief for the family or primary caregiver and can be provided in the consumers home or in an out-of-home setting.

Enhanced Respite

Enhanced level of respite also provides periodic relief for the family or primary caregiver and can be provided in the consumers home or in an out-of-home setting but is for participants who have behavioral or medical needs that require staff that are specifically trained to conduct personal care tasks or behavioral procedures.
